All volunteers are required to complete this application prior to participating in the Bridge2College program. For the safety of all participants, including students and volunteers, a background check will be conducted on all applicants. A valid government-issued photo identification must be presented to obtain the Live Scan Request Form.

Have you ever been convicted of, or pleaded guilty or no contest to, any felony or misdemeanor?

Note: A conviction will not necessarily disqualify you from volunteering. Factors such as the nature and gravity of the offense, the time elapsed since the conviction, and its relevance to the volunteer position will be considered in accordance with applicable laws.

I agree to abide by all state and federal laws, and all policies and regulations of the Bridge2College program. I understand that all involvement with the students shall be under the supervision of President, Johnny M. Baltierra and is restricted to the activities of the Bridge2College program.

I agree to volunteer my services, without compensation or reimbursement, for Bridge2College.

I understand that I am required to my fingerprints for the purpose of obtaining a criminal record summary from the California State Department of Justice and the Federal Bureau of Investigation.

I agree to indemnify and hold harmless Bridge2College, it’s officers, employees, and agents from all claims, liability, damages, suits, losses, costs and expenses for injury to my person or property, including death, and all costs for legal services arising for my volunteer services for the Bridge2College program and activities associated with Bridge2College.

This authorization shall remain in effect while I am involved in the above described volunteer service with Bridge2College.

I declare under penalty of perjury and under the laws of the State of California that I have completed the above information truthfully, have read and understand and will comply with the Bridge2College requirements and expectations for all volunteers. I understand that if the information which I provide is not accurate, my volunteer services will be terminated. Bridge2College reserves the right to conduct a criminal background check of volunteers as permitted by law.
